You don't necessarily need to start from a cube, but it's doable, if you look at Dikko3D's guide he starts from a cube, deletes the sides, scales down along X, makes some loops for eyes and mouth and then fills it in

It doesn't have anything to do with the result. Like any drawing or painting program and the final L2D model. A pixel is a pixel, a vertex is a vertex. It all depends on the person using the tools and not on the tools themselves.
