Try putting on an appeling personality and 100 percent carrying a coversation for hours, while also changing your voice, and mannerisms, then also having to have some entertainment value, lets see how far you get. Oh and you have to do that for multiple times a week and there are incel antis that want you dead for existing. They are content creators like anyone else, and in order to try and make it you have to understand what that means, you can't just sit there, you have to be making somthing worth watching. A believable reason for not officially acknowledging her back injury is that it could jeopardize her art career. If companies had reason to believe that she couldn't complete work due to her back injury then she wouldn't be offered contracts. It would mean that 'back hort' is a legitimate reason to leave Hololive (or not be offered a new contract by Cover) but something her roommate would never want to admit to for the sake of her art career.

What we do know is that Sana took a series of lengthy breaks, and came back with an injury of some kind that interfered with her ability to stream. Sana described this as an intense pain when she was in the seating position required for the motion tracking of her model to work. Since we can't independently verify this information we really only have her word for it; before, during, and after her graduation her reason for leaving was kept secret (and several members described trying their very best to convince her to stay). None of this happened on stream, so we just have their word for that as well.

After the graduation, a bunch of her fans followed her to the Twitter (and Youtube) account of the person who played Sana, and were obnoxious enough to cause her to tweet about it. That tweet has been interpreted to mean her previous statements about an injury were false, but again, we just don't know.
