Vedal runs a Large Language Model that is fed chat messages as an input and whatever she says is the output (probably). That output is fed to into a text-to-speech. Her model is just a regular Live2D with some animation running, it's not controlled by AI as far as I can tell. Playing games is achieved with different machine learning model, probably one model per every game.
Then Vedal just sits near his PC, ready to press censor button if she says something controversial.

I'm guessing it's some weird unreproducible timing issue to do with when he gets the data back from the TTS service.
There are some other things he could fix with the TTS too, by applying a string replacement filter on the text that is sent to the service, but not applying it to the text that goes into the subtitles.
For example, it's annoying to have her read out the underscores in people's usernames, so he should just replace them with silent spaces.
If he wanted to get really clever, he could try detecting when someone's name was in l33tsp34k, and replace the numbers with the relevant letters.
Having text translations for the kaomoji would help too, so she doesn't pronounce :\\ as "backslash backslash".
That also reminds me that he needs to add some additional fallback fonts to his subtitle system, to make sure it can display emoji and non-ASCII characters in people's names.

Maybe I'm misunderstanding, but there appear to be two "modes" for her subtitles, so we might be talking about different ones here.
Let's say that Continuous Mode is where the subtitle text appears one word at a time as she says them (except for weird edge cases like URLs, which appear as if they are a single word but are spoken as multiple words).
This is a feature, for the reasons you give.
The other mode (which is probably not intentional) let's call Flash Mode.
In this mode, the entire text appears on screen for a brief moment, then disappears, then the TTS clip plays (with no text on the screen), then the entire text appears on the screen at once after the audio clip ends.

Here she starts saying a comment about "I'm not sad..." with the subtitles only appearing afterwards, all at once.
The subtitles for her next comment, about "Robocop", flash onto the screen at 1:12:23 before disappearing and then appearing a word at a time while the audio clip is playing.
So I guess the line between Flash Mode and Continuous Mode is not as clear as I thought, and maybe there are two bugs: the subtitles flashing up at the start, and the subtitles not appearing until the end.
It would really make the stream feel more polished if Vedal manages to fix this, although I guess it's not a priority.

He could maybe train an AI to complete old 8-bit Atari games, as there are some papers explaining how to do that, but it would make more sense to outsource that work, or just download existing solutions.

I'm not sure if there is an objective measure of complexity that can give an idea of which games are hardest for AI to solve.
Much of the complexity of Minecraft comes from its random terrain generation, and the fact that the world it simulates has three spatial dimensions.
I suppose the biggest difference, though, is that for Dota 2, OpenAI gave the AI access to internal game state representations, whereas recent Minecraft AI research projects have tried to solve the game using only visual data.
Perhaps with a sufficiently hacked Minecraft client, Vedal could give Neuro a very formulaic AI which just has a state machine and some simple pre-trained routines to follow.

There is a bot that has beaten Minecraft already, and it did so in 2021:

The way it works is actually exactly the way I guessed, though, using a hacked client and splitting the run into subtasks.
Vedal should definitely try using that as Neuro's AI for playing Minecraft, and it should be relatively easy to use the task descriptions as triggers for her language model.

Game complexity isn't the only thing that affects machine learning. One of the big problems is 'scoring' the AI's performance. Dota is easier to score than minecraft. Open ended gameplay, and long action to payoff timelines are the banes of AI training. If the AI does something now, that will pay off much later, then does something dumb that kills itself strait after, both actions will be suppressed. If by some miracle a long term action pays off without something else interfering, it gets only a slight promotion, along with a whole bunch of actions that contributed nothing. Learning long term planning this way is a solid bitch.

Her AI was pre-trained using machine learning, whereas altoclef was "taught" by having rules hard-coded into its brain.

Her ability to react to videos is activated specifically when she is *not* playing a game.
I think the diagram needs to make that clear.
Also I'm not sure if the language model uses a transformer architecture (like GPT), and it might use an RNN instead.
One thing that I've not seen discussed is the difference between her responses to chat messages, and her seemingly out of the blue rants / stories (like her "my girlfriend..." or "bullets per minute" comments).
There are also her "Sometimes when I stream, I envision myself..." comments, which seem even more formulaic and prompt-engineered.
It's possible he uses something like GPT-3 to pre-generate a bunch of rants (or at least ideas for rants) and then drops them into the stream at random, or whenever she hasn't found a good comment from the chat to react to.
