How do companies like Hololive and Nijisanji prevent incidents where the facerig software bugs out and shows cam?

Seems risky to run a business around virtual youtubers/streamers with software prone to this kind of error

Actually the baseline VTube Studio on iphones just sends positional data for face parts to the PC it is hooked up to, so literally no camera feed is ever touching a PC. Then the software on the pc just uses that positional data to know how to move the model, and the model is what is put into the feed. Showing a face shouldn't even be possible in any decent modern live2d software.

Because they know how to use OBS. If you've ever streamed enough to use it regularly you'd know that not including the camera feed in the first place but still keeping the model is an option. This retard probably kept the camera feed on a preset or as an invisible layer and pressed the wrong button.

Hololive proprietary software doesn't use the camera, only the "face detector" sensor that Apple uses to, for instance, power their "faceID".

It doesn't really capture any image, just the biometric position of eyes, nose and mouth

This. The input (camera feed) and output (model) are completely separate from an OBS perspective, and a pure Vtuber who does not do face streams would simply never include the camera feed. Any software bug/crash would simply result in the model freezing or disappearing (which does happen occasionally).
