The only thing I consider a 'fake' voice is if they sing in it and can't do vibrato no matter how hard they try without significantly changing their voice (see: Towa). Try it yourself. You can't create pleasing, unforced vibrato if your throat is tensed or held unnaturally in some way.
Everything else is as 'real' as the rest.
