Since most of the replies leave out parts of the story here's what happened:

Her first fuck-up was before her debut. She tested her model during a public stream on twitcast.
When she debuted she sang happy synthesizer which made it easy to find her past life (even I have had song covers of her on my drive). Nene following her on past life's Twitter surely helped. But knowing the past life or roommate of any holo is no reason for a graduation. The issue was her past life itself.

People figured out she had a boyfriend thanks to her steam profile. She said they broke up before her debut to please idolfags. But additionally she was also known for feeding private stuff from Kudo Chitose from Nijisanji who she knew from her past life to antis, which made Chitose eventually graduate. However, in her apology video she did not mention the issue which made Nijisanji fans even hate her more.
Meanwhile her whole private life was a mess with her stepfather harassing her so she was a natural born menhera from the very start.
Cover decided to stop her activities for a period of time until things settled but she decided that she hates all the companies and the management for doing that so she quit all together and never came back.

Some fun facts from her time before joining Hololive:
She recieved a dildo as a present which she cut up into sashimi as a joke for christmas.

Kiara was more succesful than Mori, sorta. Succesful enough to make a living out of it, at least, whereas Mori had to take an unrelated teaching job.
