When you're looking at combined metrics for the US, you're getting a total picture. That includes all of the free clinics and reduced cost facilities that have overstressed, overworked, and burnt out doctors. Most Americans are even painfully aware of which hospitals in their metro area are the "poor" ones, "regular people" ones, and the "rich ones." All of which provide vastly different qualities in health care.

If you're rich; America is the best. Unfortunately for many, it can also be one of the worst (at least in regards to 1st world countries.) That's why you see most metrics landing America somewhere in the middle.
