She died when the ghost matter exploded. All 5 planets where engulfed in matter, so their version of the moon also got engulfed. The Eye was too far from the interloper, so it wasn't affected by the matter.
Solanum died in 5 versions of the moon, but stayed alive in the other. However, since she was in the quantum moon, she herself was "quantum entangled", so her alive version was only ever alive when someone else was there to observe her, so time didn't pass for her

There's all kinds of weird shit that doesn't really make any sense. You just have to accept that it's a game.
For example, Fauna herself pointed out that corpses should have eroded long ago, hence why she believes the Nomai to be relatively recent arrivals. Why are they there this long after? (And don't say "lol vacuum of space", there are corpses everywhere, including in areas with atmosphere and directly under the spray of sand.)
Also, red giants don't work that way, and neither do supernovas. What do the anglerfish eat? Nothing makes sense, but it's still a great game.
