I would assume the incest parts might be too controversial too tackle, compared to the more "common" themes like gore and cannibalism (ridiculous and hypocritical if you ask me but w/e). Also only 2 out of the 4 episodes are out, with the rest coming next year, so maybe some are holding out to play the whole game all at once.

Note: I've played both episodes and I'm (I think) on the incest route because incest is wincest, but there's no doubt the topic is still controversial and may not get approved to be shown to the general Holo audience.
That said, they played that streamer some game eventually so there's some hope.
