Arguably only really deep southern accents or appalachian can be hard to understand, but generally American-English is pretty easy to understand. Midwesterners especially- news reporters from America/Canada often times spend some of their careers there to learn the accents so they can be more understandable. The Nebraska/Iowa area especially has arguably the most neutral accent of the English language.
