i have a 21 inch tv which i sit 7 feet away from, and sitting further away is not an option, playing games in 4:3 makes the screen to small and my eyes get sore, so i stretch them to 16:9, which makes the games look like shit but makes them playable.
how do i make my eyes not hurt when playing my pc engine games at 4:3

Real hardware already stretches the games out to a slightly wider aspect ratio to fill a 4:3 CRT screen.

But no, even with my 16:9 BVM, I still play games in their original correct aspect ratio.

If widescreen is actually an option (like DK64 or Perfect Dark), then I'll go with that, or use a widescreen hack for PC games and emulators where it's available. If it's just not an option at all, then I'll stick with the original aspect, because stretched a shit.

You can extend the screen drawing in bsnes hd in Retroarch so it's true widescreen, although the sprites disappear outside of the 4:3 region. Works well on mode 7.
