It's clearly not intuitive and was the most popular reason for calls into the Sega help hotline at the time it was released, before zoomers were born. Pressing up or down isn't something that makes objects move in Sonic any more than you would expect it to make objects move in real life with momentum and jumping instead being what moved things, and you don't get feedback of pressing up or down doing anything unless you start doing both in alternation for long enough to get it going making it extremely easy not to see.
The "arrow" excuse is stupid too. There's a diamond design on the barrels that just look like some circus stylization, not arrows.

Its not intuitive because jumping on the barrels moves them up and down and all earlier barrels can be effectively passed by jumping. So when you eventually get to one that jumping will move but not far enough to reach the passage, its confusing as shit because the game taught you that "jump = progress" earlier in the level.

The only way I was able to pass the barrel was by glitching through it. My brother discovered this method by jumping on the barrel a couple of times until it got low enough, short jump, transform into Super Sonic, and the barrel would pass over Sonic and push him down below. Because of this, we could not beat the game unless we had all the Chaos Emeralds and had no shields, and at least 50 rings by the time we reached that area.
Since Tails couldn't go Super, we weren't able to beat the level as him.

In every game made in that era, if you could not get past at some point, you tried every other combination of buttons/keys to see what they did. "Does pressing up makes the game do something here" or does press down does, etc. That's how I played every single late 80s/early 90s title, especially PC ones. The way you describe the barrels makes me think that if it weren't for the manual, you couldn't even get past the Start screen of a videogame.

The jump only throws the barrels out of equilibrium so they CAN move, after that you press and hold up and down. The fact that you get stuck to the barrel itself will hint this. I genuinely got stuck at this a few years back and remembered it was a meme but not the solution.

The problem stems from it bouncing when you jump. So it feels like it's you have to spring off it or something or time your jump off it. You never get the feeling you can control it. Nothing like that is ever done in any of the first 3 games.

In fact, my initial impression is like those little springs in Sonic 2 casino zone that you hold jump on to spring off of. I though it was a jump timed platform.

I was a kid and couldn't figure it out. It sucks that I couldn't play the rest of this expensive video game my parents bought for me because I was retarded. It was supposed to be a simple platformer I could enjoy as a 5 year old, not all of a sudden some puzzle platformer. There just wasn't much feedback from pressing up and down, I might have tried it but all I know is I never noticed a difference and that was the end of playing Sonic 3 for me. What you have to understand is that at that age it's not like I played many games before. The biggest reason the barrel is stupid is because it sets you on the wrong mental track.

Jumping on the barrel gives you instant feedback.

You encounter barrels like this earlier in the level that can be navigated successfully by jumping on them.

When you get to this barrel you can get to just a hair shy of where you need it be by jumping on it.

This puts you in the mindset that you're on the right track but you're just not doing it well enough. So trying other button combinations, ones that don't even give you immediate feedback--is unlikely to even enter your mind. You're just left to think this barrel is bitch hard compared to the earlier ones.

There is nothing intuitive about the barrel because Sonic controls based on the application of force but pressing Up and Down to move a barrel like an elevator isn't about force but some kind of magic. When you press the Jump button, Sonic pushes off the ground. When you hold right, he runs right. When you're on the Metropolis nut/bolt gimmick, you move it by running along it, applying force to spin it the other way. But the barrel? Why does it move up and down while Sonic is simply standing on it? It doesn't make sense and it breaks the "rules" established in every other Sonic game.

Intentions matter. Sonic is a children's platformer, really designed to be a lot of kids' first game. The health system is very generous, every button (A, B and C) on the controller does the same thing, it's a fun and straightforward title for kids. Not so. The spin dash is an ability listed in the instruction manual and the player knows that they can move Sonic with the controller to destroy obstacles (bad guys, walls, etc). The problem with the barrel is that it isn't about the application of Sonic's movement or force at all. The D-Pad moves the barrel as if by magic while Sonic is standing on it, he isn't pushing it up or down. The controls for 99.999% of the game move Sonic, and in this case, they move another object. You aren't solving a problem by moving Sonic, you get through the barrel by somehow controlling something else...something that doesn't give immediate feedback either when you do press up or down.

Kids are aware of their limitations. It's easy for a child to play this and think they just aren't good enough at jumping at the right time, that this is a problem of timing since jumping does provide immediate feedback of moving the barrel. That was my problem when I played it. I even played puzzle games like Myst at that age too, it's just in those games I knew I was supposed to solve puzzles. This was an ill-defined problem, it isn't obvious at all another solution is required besides good timing, a lot of kids myself included just thought they had to get good at timing jumps on the barrel.
