I don't know if MK sold much in Japan. Never seen it in the lists of best selling games in Japan, for any platform. Never heard any praise for MK from Japanese either.
I've checked GameFAQs, seems like MK3 wasn't even released in Japan on SFC or MD at all (only PS). And after 3, they just stopped releasing MK there altogether. Looks like it wasn't very popular.
I guess Japanese just looked at MK like some Western kusoge. Something tells me a violent game for American dudebros wouldn't sell there. Plus, I don't think many Japanese games used digitized sprites. Not sure if they liked those—probably not as they mostly stuck with anime style. They weren't too keen on FMV either.
Also, I'm not even sure Midway exported their arcade cabs to Japan, and Japan had more than enough of their own arcade games.

It's the same basic art as the US version but the box orientation and composition makes it look nicer. Feels like a banner or scroll that would be hanging at the actual tournament.
