What is causing brain waves is changes in voltage potential of populations of neurons. That can be artificially induced by magnetic induction, a technique which is known as transcranial magnetic stimulation (TMS), or by transcranial alternating current stimulation (tACS). Good question, and to be honest, there's no clear consensus. The functional significance of oscillations is still a matter of ongoing debate, but and there is convincing evidence either way. Like I mentioned before, Buzsaki et al have argued that theta/gamma coupling and phase precession govern encoding of spatial information. Genetic deactivation in mice of the expression of the mechanisms that lead to the development of GABAergic interneurons (which drive gamma rhythm) leads to an abolishment of hippocampal gamma. This in turn leads to severe decrements in spatial navigation of these organisms. Optogenetic entrainment of hippocampal gamma and theta on the other hand leads to improved spatial navigation performance, which is a strong argument in favor of the functional relevance of these rhythms.

On the other hand, some have argued that oscillations may be merely epiphenomenal (a process which is the result of some other, underlying, process). Medial frontal theta rhythm for instance has been related to cognitive control and behavioral adaptation following errors, however, transcranially induced theta rhytm doesn't seem to improve or impair performance on tasks which measure this.

TL;DR: the evidence is mixed.

As it's commonly viewed, yes. Most people define 'functional connectivity' though as nothing more than two regions covarying in their neuronal responses, which would be equivalent to synchrony. That doesn't say much about the actual, direct, coupling between regions though. For instance, region X and region Y may show the exact same oscillatory response, which would make them perfectly synchronous, and thus by definition, functionally coupled. It is still possible though that there is no actual information flow between region X and Y because they are both driven by region Z. This has led to the development of techniques such as Granger causality which infer directionality of information flow by temporal precedence. The justification for that would be that if region Z is responsible for the covariation of region X and Y, there should be little temporal lab between the oscillations of X and Y. If however, there is no region Z, and region X drives Y, then region Y should lag behind region X in its oscillations. Furthermore, the activity in region X should be predictive of the activity in region Y to a greater extent than the previous activity of region Y. With that in mind, there is substantial evidence that synchrony between DLPFC and MFC on the one hand, and visual/parietal cortex on the other hand, mediates cognitive control during the stroop task.

What are you looking for? A book which describes methodologies and data analysis? To my knowledge, there's not really a good book on the market yet which explains anything beyond ERPs in depth. A little bird told me though that Michael X Cohen is writing one at the moment though. It'll be presented at the next SFN conference, and will be for sale after that.

Supposedly it works by entrainment of oscillations via the auditory system, but the consequences of this entrainment are limited. Intrinsic neuronal oscillations are the product of specific cellular mechanics, each with their own topographical and frequency signature. Entrainment doesn't necessarily recruit the same processes simply because there is exogenous drive. The driving mechanism is different, and so the tonic neurochemical substrate is left unaltered.
