Runaway power increase from boiling off some of its neutron absorber causing more heat to boil off more water etc. When the vast majority of your neutron absorber is water then this can become an broblem pretty quickly and you're reagdor is ded :DD

Turn of all fail-safe systems, poison it with xenon by low-output running for a day, pump too much water in, pull all control rods out. Disconnect the pump form external power source and let it be powered only from the leftover energy of the reactor WHICH DOESN'T FUCKING WORK. Now the reactor work with any actual cooling, diminishing the xenon, boiling water to vapor, also producing hydrogen, overheating the control rods canals so they can't be used stop it (no, AZ-5 won't actually save you at this point), all these having positive feedback on the fission, increasing the pressure to the point it ruptures. Now oxygen can go in, oxidizing the hydrogen and superheated carbon in graphite moderator. BOOM
