tbh not the greatest novel I've ever read but certainly disturbing. Almost Transparent Blue is a way better book by the same author but I don't know if it would be disturbing enough for you, it's bizzare and fantastical writing about sex and drugs with a great orgy scene. One of my favourites. A great disturbing writer in my opinion is JM Coetzee, specifically the books Waiting for the Barbarians, Dusklands, In the Heart of the Country, and to a lesser extent Age of Iron and Disgrace.

Those books have torture, rape, massacres, patricide, miscegenation, apartheid riots, post apartheid racial conflict ie "Farm Murder" and all sorts of gross shit. Waiting for the Barbarians is one of the most disturbing books I've ever read, it's enough to make your hair curl. Although maybe I'm just a pussy. I couldn't eat chicken for a while after reading In the Heart of the Country.
