First off, I'm curious if anyone has read this book and what they thought of it.

But in general I was hoping to get some thoughts and recommendations on incest in literature. Not necessarily the typical "it was bad, violent trauma" type narratives (I suppose pic related might count but it was more nuanced than that), but any sort of depictions of it, whether it be romantic, idealistic, philosophical analyses, etc.

I wouldn't say it's hard. It has no pyrotecnic narrative techniques nor a hard prose. It's told like a book of memories from the POV of the brother about his childhood and early teenage with his sister. There's also stuff there about music and Italian Opera.
Author loves his Faulkner too, you'll see.

There are tematic and plotwise reasons for the name. But even knowing that I find it weird too the chose of the name for the heroine of the story. I don't know exactly what the author was thinking.

Yes. Nahuatl is not spoken in Peru. Peru has it's own indigenous languages (like Quechua, Aymara and the multiple ones from the Amazon Rainforest).
