Do you ever take notes either during or after reading a book about your thoughts on the text, such as the subject matter, personal insights, or the quality of the writing itself?

I typically take notes within the book or in a notebook as I read it, but oftentimes I'll sit down after finishing it and write about it to sort my thoughts

Yeah
Most extreme i did was make a summary of an entire book. It was Jacques Ellul's work on Propaganda.

When I'm not lazy I do. Those books are always the ones I enjoy the most and get the most out of. The key is when you feel like you just read something really interesting, put the book aside and pace or go for a walk until you really get to the bottom of why it was interesting for you. You can generate a lot of insights this way.
