Asides from generic manga pages, GIF animations, the Tarot, and image replies, everything is either in one of two folders.

The main directory is first divided into subdirectories based on location (Forest of Magic, Hakurei Shrine, etc..). Within each location subdirectory are other subdirectories pertaining to each character known for that region (\Forest of Magic\ -> \Marisa\, \Alice\, \Rinnosuke\). In addition, frequent character combinations are also made into subdirectories (\Alice and Marisa\) and, after all that, there is a global catch-all called \group\ for non-binary character combinations from that given location.

I also have two a subdirectory for \Youkai\ into which the Touhoues that have no specific location folder are filed. In \Youkai\, \Scarelet Devil Mansion\, and \Hakurei Shrine\ location-folders, I have \Survivng X\ folders, where X is either Cirno, Flandre, or Suika, respectively. These folders are "X and someone else."
(Note: Suika is improperly listed; move to \Youkai Mountain\ location.)

After all that, I have, back in my original directory, a folder called \Group\ into which any picture with two or more people from different location subdirectories are stored.

I save almost everything, in bulk, to the major folder \New\ before sorting.
