I think the general interpretation is that the extra arms are split bones, blood or bits of broken care/general messed up shit. Especially if it's gore, the subconcious of a young girl seeing it might rationalise it as her sprouting extra arms.

Of course people only assume that because of the stoplight, and getting it from a traffic accident victim. But the spotlight also has other weird effects, like showing the invisible girl in block world. Maybe it has the effect of showing someone's true self, or something, and she was a cute girl who was monstrous on the inside.

The way she bounces around and Techno plays is what makes it surreal in a frightening kind of way though.

I almost wish we were given some clues to go with(just not during the main game itself) as it would make the theories a bit more interesting.
