Alice is explicitly stated to be a human turned magician in canon. With Patchy it's never mentioned how she became a magician so I just assume she was born that way.

Supposedly there are two sorts of "magicians" among Touhou youkai, that besides the name and association with magic have little or nothing in common. One sort are born magic wielding youkai, the other are humans who are transformed into supernatural creatures as a side effect of their trade.
