it's rare that I do but when I have to leave I try and clean myself up, unfortunately all I own are sweatpants and t-shirts which is not usually considered "normal" attire.

Plenty of hikikomori are capable of going outside. In fact in NHK ni Youkoso!, Satou mentions that he can (only) go to the store late at night when nobody is around. I am the same way. I think I would be considered a hikikomori if I was in Japan.
