The difference is mainly the fact that with/without eyes is often a side effect of a much larger decision made by the eroge's creators; whether to make the protagonist a person or not.
The outcome of that decision ends up whether you see the story through the eyes of a character with an actual personality, actual motivations etc. or a sockpuppet that just wants to rape his stepsister.
