Mods are not limited to one board. Some of them just have pet favorite boards, and hence they look out for them more.

Of course the really big boards are pretty much unenforceable for a lot of their problems (such as /a/ and /v/), but that's more a matter of there simply not being enough mods/janitors for such populations. For example, 2ch has >300 mods. That's probably 100x as many as there are active mods on 4chan right now. With enough manpower moot could even wrangle /b/, it's just he doesn't care enough about the site and hides behind the durr anon is a free speech movement shit to cover for his poor management.
