Is it possible to make custom scenes/levels for any of touhou games? Can be both shooting ones and fighting. I would love to make my own. And would it be possible to add custom characters?

You would have to reverse engineer the stage formats. I don't think anyone ever did anything but a superficial patcher for some of them.
Possible if work is done? Yes. Possible now? Not, unless someone wrote some tools for them.

There are some open source danmaku engines which you could modify instead.

You can make decent danmaku games in Gamemaker if you can get your head around the language included. Even at minimum requirements Gamemaker 5 supports 32.000 moving objects without noticeable frame drop.

I suppose it has the exact same problem as mine. I only gave names to a few instructions (only those that I cared to understand what they do and that I wanted to modify). To actually make it useful, you'd have to spend a lot of time experimenting and reverse engineering ZUN's code.

From what I've seen, ZUN's Embedded Common Lisp abstracts a lot of game-specific logic.
For instance, there is an instruction for spawning multiple linear-moving bullets at different speeds and otherwise identical parameters. I think it might be abstraction inversion to use such a bytecode for non ZUNish danmaku.
