Are there any arcs in Higurashi in which Mion and Shion didn't get their identities swapped around because of the tatoo/heir apparent business, or is it sort of a given with varying degrees of crazy as a result?

An interesting question. As I see it, there's a few possibilities:

1. As evidenced by the arc in which Akasaka's wife didn't die, it's possible for Rika to appear in a world with changes that occur before her actual appearance or awakening or whatever. Thus, it's possible for the Shmion confusion to not have occurred.
2. Unless it's impossible for changes to be made to events that occurred before Rika's birth, which, if I'm not mistaken, is the case for the Shmion confusion.
3. But evidence against point #2 exists in Meakashi-hen, what with Rika never becoming Hanyuu's miko because of changes to events that occurred before her birth. Given Meakashi-hen's special nature though, it's possible that such a thing was only possible in this very specific case.
