Lunarians are planning a second invasion so Yukari gaps you into space a few miles out from the moon. You have to sneak your way past Lunarian defenses. The game is kind of like Seihou because Lunarians fight with high technology, so you're dodging bullets, shells, missiles, and concentrated magic weaponry, and fighting armored troops instead of fairies. The bosses are either military officials or magicians. The last area is the Lunarian capital, and the final bosses are a gauntlet of elder Lunarian sorcerers. Upon confronting them you find their goal was to open a gateway to the demon world somewhere on Earth, hoping the demons would overwhelm and destroy the Earth and thus Gensokyo along with it. The EX boss is a greater demon that the sorcerers were planning the invasion with. Player characters are Reimu, Marisa, or Kaguya.

Underwater Touhou.

It would be very different, because faeries can't fly underwater, and Reimu and Marisa can't normally breathe there. The basic enemy types might even have to be changed.

The gimmick might be air points, which give you time to let you fight against a given wing of basic enemy types. That's right, TIMED STAGES.

Reimu fights another youkai as usual. After defeating it, she realises it's been secretly helping her maintain the barrier.

Youkai flood out into the real world, JSDF and elements of the US military respond with full force. Bullet hell becomes literal as the player must dodge fighter jet cannons and missiles, and restore the barrier before humanities one final nuclear-tipped FUCK YOU GENSOKYO gesture makes it through.

Call of Duty: Gensokyo Warfare.

Instead of the usual main characters, you choose one of 3 new touhous, who are all prisoners in hell. The heroines are given a chance to escape hell by completing a challenge. The challenge is to capture a special ghost cat and return to its owner. This will of course require fighting through 6 stages to catch the cat itself and then an extra stage to return it. Oh, and the stage 5 boss's last spell card will have a very short time limit and kill you if you let the time run out.

Expand on Hisoutensoku and create a game where Hisouten goes out of control and runs rampage on Gensokyo. Would be cool to have a giant robot doll as the final boss instead of another new loli.

ZUN could make one large game with totally unrelated stories. There are 4-8 playable characters.

Each character's route is related to their past, parents, upbringing, how they came to be alone, etc.
For example, Reimu would fight enemies that saw her grow up, like Yukari, among others. Perhaps Yukari would be the final boss. Or maybe the spirits of Reimu's parents. Who knows. The conflict of Reimu's story could be related to the border at the Hakurei shrine weakening or something.
I don't consider myself an expert on Touhou storylines.

You play as Sakuya, but instead of controlling her, she controls the fairy maids against Marisa, who happens to be invading. You click to direct shots, and have different buttons to send different waves/death fairies.

In essence, a regular Touhou game where you control the "enemies" against the protagonist (CPU or player 2).
