Being a Touhou heroine actually means putting your life on the life.
You can only take a few hits, you can only use a few spellcards, and your danmaku is limited to a forward beam-esque pattern.
Enemies gain power from the incident and can't die. But Reimu, Marisa, Sakuya, Sanae? Once they get hit a few times, they WILL die, because the incident doesn't lend them power.

Besides Mokou, nobody really dies in a spellcard battle.

The only part that could be interpreted as a death is when midboss Sakuya explodes without further explanation in Remi's stage - but since Sakuya appears again in PCB the point is moot.

The whole point of the Spellcard System is that the winner and loser both survive. This is as much to keep weaker humans alive as it is to keep the youkai from wiping themselves out.

Where do you think the level 6 bosses get their elegant houses from? It's not like there are any masonry youkai. They need the humans alive to support their lifestyle.
