But Kurumi has no proven connection to Makai. She lives in or close to Mugenkan presumably. She's probably just a regular upper middle class or lower upper class vampire, based on her attire and possible living arrangements.

Elis on the other hand is a genuine Makainese vampire. Not only that, she's one of the more elder daughters of Shinki, the creator and Goddess of Makai, so as far as the Scarlets are concerned she's basically foreign royalty. Remi in particular would bend over backwards to accomodate her, just so she could show off her connection to her to the other important people in Gensokyo like Yukari, Eirin or Kanako.

Mugenkan is meant to be at the very least in the borderspace between Makai and Gensokyo, the lake of blood appears to just be a shortcut.
Seeing that it's a hop, skip and a jump away from Makai, and the LoB is in a very difficult place to get to conventionally, I would think it more likely that Kurumi came there from Mugenkan, probably first from Makai.
Besides, anything else would destroy my headcanon of Elis sometimes visiting her distant relative, which is too cute for me to not defend.

Although I would agree that Kurumi would talk with a rough country bumpkin accent. The gap moe between her outdated and provincial speech and the little blonde gremlin is also very cute.

Kurumi being related to Elis to the degree that her place in the Makai line of succession is so far down the ladder that finding her exact standing would be only of academic interest makes more sense. She would be free from Makai politics and Shinki's expectations, but would still likely have some perks that her ancestry has afforded her, like a comfortable lifestyle and high class clothing that she seems to be wearing in LLS.

And Remi would no doubt still feel somewhat superior to Kurumi for having less diluted pedigree at least in her eyes. If Kurumi were considered gentry, Remi and Flan would be peers in comparison, while Elis would be royalty.

The rules are different for different classes. When a highborn noble like Elis acts in a socially unconventional way she is described as "an eccentric", "bohemian" or "a character". But if a commoner like for example Reimu acted in a similar way she would instead be described as "a drunk", "ill-mannered" or "a disgrace".
