Even average Japanese wouldn't be able to answer, any more than average Americans about American beauty standards and their contextual differences. Not being Japanese (or American), I can only observe general theories from interpolation. Differences in contexts exist, however, common tendencies are notable and I believe likely universal.

At least here, in ads and other consumer direction, moles and such are signified as a part of personal beauty because they are relatable blemishes for average people. "You see, even that beautiful model has moles?" That kind of thing. In the past, before cultural changes, for celebrities these could become charm points and even sexualised, so at different times some "imperfections" were desirable almost as fashion trends. Some are more common and lasting than others, even existing at higher levels than any particular cultures or countries, while others are more localised and ephemeral. However, for all this, let's keep in mind the idea of personal beauty.

A singer in the eighties had a prominent mole on her face, so at least one generation of men grew up divided. Was her mole a charm point or did it ruin her beauty? Here this kind of thing is no longer discussed in "respectable" public discourse but, to this day, you can still see it in Japan even if maybe also less than in the past. I use this example because even at the height of this singer's fame, her album covers obscured her mole. I don't know if it was ever edited away, but certainly hidden by an angle or hand or flower or such, despite many seeing it as a charm point. If anybody would draw her, it would have been important to draw the mole because it was an important part of her personal look. This is even true of average people: if a prominent personal feature exists, it will be drawn. Isn't this universally true in every country?

With photos, usually it's easy to tell people apart from each other unless they're lookalikes or the angle is strange or other contextual reasons. But, in drawings, the less realistic the depiction, the harder it is to say who it's meant to be. For this, exaggerated personal features are drawn. When creating fictional characters, it makes sense to maximise differentiation. Moles are one of many easy realistic solutions. If the media is erotic, then some erotic focus on the exaggerated personal features is even to be expected, right?

Now let's look at why this is not the case in JAV covers and other photos. Why would the personal beauty, in other words charm points, be edited out or otherwise obscured? Probably this has two reasons. First, of course the default assumption is that people can tell people apart from each other. Their personal beauty is not as important in this case, because they are still identifiably the same person. It's true this can go too far, surely we can all think of examples from JAV covers, but the base assumption remains because usually it does not too far. Second, a marketing decision. We all know what beauty standard is the lowest common denominator of standard beauty. We also know the desirable sexy body types, though, much variation exists and for this the same bodies may be edited to look very different for different contexts, such as ass fetish or loli fetish in JAV. More men will be interested in a JAV if the cover looks like an "ideal woman", which varies by context but, in most cases, conforms to the standard beauty or one of common sexy body types. It is more risky to include personal charm points, as for most men it's not so important either way. If he bought a JAV, he probably will not demand a refund just because she has a mole. If he sees it as a charm point, then he will know it's edited away in photos. But, it remains in the JAV themselves, so it isn't so important. It's the same with bodies. Maybe her body was edited to either more extreme or more standard in different contexts, but usually not enough to be a "dealbreaker". Exceptions may occur.

For some JAV actresses, their highly unique look could never be edited away and even the covers reflect this by not attempting to conform her to standard beauty. However, even in these cases, "blemishes" often would be edited, at least because of two reasons. First, the risk of turning off men who really don't care but will not see them as charm points. Second, in photos they may stand out more as "blemishes" than in videos because everything is a bit edited, so even men who will see them as charm points may be turned off. JAV covers must maximise appeal. For this reason, editing away "personal beauty" towards "standard beauty" is practically understandable.

As for why personal beauty may be even eroticised in non-photographic contexts, another reason may also exist. If you were her boyfriend, you would eroticise her unique features, right? Or, even if you only saw her once on the street. These are things you may remember about her, if she has a prominent mole or such.

I appreciate knowing it was not perceived as too autistic. Really it's because of the character limit, without it that post would have been more autistic. So, I decided to trim the autism and focus on observable tendencies and logical speculation.

One autistic speculation I wanted to include but left out is the concept of projection.
