I'm Asian and can often guess the difference if they have very stereotypical features. Korean women with plastic surgery have a certain eye shape, and Japanese women often have a particular kind of double eyelids that are not replicable even with plastic surgery (e.g., Korean women can't get the same eyes). Aside from that, there are other features that help distinguish general regions. Like I can tell Vietnamese from Thais or Cambodians or Filipinos to an extent.
