It's rather far off. I'm suprised I found it by randomly wandering.

To get it:
1. Enter the gravestone world and eventually you'll find one of the giant shapes that has toppled over and vomited.
2. Go through its mouth and head to the left side of the area and go through the wide door.
3. You'll be in a waiting room-like area, and head left through the next wide door.
4. You'll enter a series of hallways with three doors and another wide door at the end. All of the three doors lead to hospital rooms that sometimes have random stuff in them. Keep heading left.
5. Eventually you'll enter a hallway without a wide door at the end of it. Enter the first of the three doors.
6. You'll eventually come across an open area with a moving background. Head and search around the bottom of this area until you find a red portal.
7. In this area, you'll find those weird shapes again, not all of them have fallen over/thrown up/whatever. Walk through the mouth of one of the shapes and you'll find it. It's not really hard.

I started playing this and, being a big fan of the original, I'm quite impressed that they've managed to emulate the feel of the game. There were a lot of speculation as to what the backstory of Yume Nikki was and if her dreams meant anything and this one has a lot of the same vibes. So far I've really enjoyed the hospital scene with the bed and going to the giant organs by the girl in the theater room.
