She often makes morbid comments too. People tend to downplay this with the usual "banter doesn't mean anything, it happens all the time in Touhou", but they forget that:
1-This type of banter is meant to be directed at the opponent, usually pre-battle. Koishi does start her battle by saying that she wants to decorate the entrance of Chireiden with Marisa's corpse, but she also says that Reimu uses her powers for boring ends, while she would "start a love-stricken massacre! Or maybe pillage from a bunch of peaceful humans". This is said after she's already been defeated and doesn't have anything to do with intimidating her enemy.
2-The creepy comments aren't an isolated case, limited to her debut. They in fact happen with a certain consistency: there are a handful of victory quotes in HM, ULiL and AoCF where Koishi casually mentions how everyone dies in the end (once in HM, twice in ULiL), that rabbits eventually die if they get too lonely, that she steps on her defeated enemy's corpse as she tries to advance into the future and that she wants to test out Mokou's immortality.
3-Precisely because of her subconscious-driven nature it's unlikely that she's "lying". Sure, it means that she blurts stuff on the spot, but it also means she's not above these actions.

Fun fact: sukima-onna is Reimu's urban legend in the fighting games, since she was the closest thing to Yukari in ULiL. When AoCF (re)added Yukari, the latter "had to settle for" teke-teke.

He's also nifty in just how much his interpretation and portrayal of touhou changed over time: starts off doing cute and chubby humorous stuff, then a bit more grim and depressing with things going more gaunt and surreal, eventually hits nightmare mode with some homage to/influence from Nightmare Before Christmas, then starts to lighten up a bi with more humorous takes on the characters' and franchise's oddities. When you start getting into the lore, the 'cute girls doing cute things' facade dies pretty quickly.

Yuyuko's suicide, Remilia and Flandre being turn into vampires at a young age, and what that implies, Koishi closing her third eye, Layla Prismriver, Mokou's death wish, the forgotten poison doll, Nue's rape and of course they all eat people.

It's the kind of scenario I imagine where you wake up in a pitch black room, completely unaware of where you are or what has happened. With a single dim light emanating from what seems like an endless corridor, your only choice is to move towards it. As you approach the eerie glow, your sense of time seems to become distorted; the light neither draws near nor does it retreat despite your attempt. Just how long have you spent seeking what seems like your only salvation in the surrounding darkness? With panic beginning to set in, you sprint towards the light, becoming desperate in your actions. Failing to close the distance, you succumb to your fatigue, falling to your knees and gasping for air. Closing your eyes in frustration, you feel completely abandoned, failing to understand why you're in this situation. But just before you give in to the hopelessness, you feel a wave of warm light wash over your body. A woman, gentle in appearance, seems to have been waiting for you. You attempt to speak out to her, but no sound is heard. It's as if you have no voice. But she understands you. She smiles, and makes a gesture to sit down in the chair across from her. Feeling exhausted from your journey, you realize that this woman isn't someone to fear. Her smile seems inviting, her demeanor soothing; you can't help but feel drawn towards her.
With no answers to why you're here, you've begun to desire nothing but a conclusion to this story. As if under a spell, your body stands up and approaches the open chair.

In a dreamlike trance, you eagerly make your way to the chair. But a sudden motion jerks your consciousness awake. Behind the woman and the chair, you see strange writhing movements. You suddenly realize that beyond this lit area is a darkness, blacker than anything you have ever witnessed. You feel chills shoot through your spine as you attempt to pull away, but your body ignores your commands. You unwillingly take another step forward as your attempts to scream are in vain, only silence escaping your throat. Disembodied hands form in the abyss, reaching out towards you. You feel all warmth drained from your body and you feel tears run down your cheeks. You glance over at the woman, desperation in your eyes, but she only returns the same eerie smile from earlier. There is no longer a comforting feeling from the woman; only an uncanny apathy towards you. The hands violently rip the chair apart, taking it into the seemingly non-existence that lurks behind the curtain of blackness. With only you left to appease the grasping hands and the devouring darkness behind it, they reach out towards your body. Refusing to accept your fate, you expend all your willpower to take control of your body once more. Twisting around as you prepare to flee, the woman stands before you, transparent with only blackness behind her. She extends her arm, offering to take your hand. With the overflowing feeling of warmth overtaking you once again, you confusingly begin to reach out, but before you clasp your hand with hers, you feel your body being pulled from behind and everything, including the woman, turns black.
