boxes get crushed like this because of one of two reasons
1) companies like to use very cheap boxes (I've seen boxes marked fragile that couldn't even support their own weight) to ship their goods in for god knows what reason. And this REALLY doesn't help with things going over seas where they will be handled more than once.
2) The people who unload and reload packages from shipping contains don't give a damn about regulations (both the ones they are supposed to follow in house and the federal ones). I've seen stuff come down the sorting belts that is way too heavy for being there and crush the hell out of packages (like in the two examples above)

I'm beginning to believe this depends on your location. For example, I live in New Zealand, and I order from Play-Asia, HLJ, Yahoo Japan, etc, all the time. They arrive perfectly. In fact, I don't worry about them arriving any more, since I know they will. I'm actually getting tired of opening packages.

From my experience in working with the Swedish mail system, that package and it's contents would have been refunded entirely. Sure, it takes a month or two to traverse all the parts of the logistics department to actually get money, but everything has insurance.

DHL/UPS/etc is for when you can accept that your package gets damaged or lost in shipping just to save the insurance money.

While working at a microelectronics service center the mail truck gave us a full load twice a day. Nothing ever came with UPS or similar shippers, but we sent stuff with them to save that single dollar. I reckon we wasted it on quadruple packaging and discreete labels

It depends on how closely they follow regulations on what boxes get used and how freight is handled. Its not really were you are located, but if you happen to be located in a place and using a service that is doing things right, keep doing it till they stop.

I can tell you how to avoid fuckups on packages you send, but not on how to avoid them on what you receive. EMS has worked for me in the past, but its not always an option. USPS goes through UPS now. DHL will be going through UPS (not that they were great to begin with). and FedEX sucks as well.

To get something shipped out of Italy, and have it make it out in relatively decent condition, I usually have to tell people exactly how to package things.

-rules of thumb for making a nigh indestructible package that will get where its going-
#use very sturdy box
#put an address label this box that tells where its going and where its from (not necessarily a waybill)
#cover this label with packaging tape
#reinforce edges of box
#wrap outside with bubble wrap and then wrap that with plastic wrap (like a trash bag)
#wrap the whole thing in packaging tape
#put address label from shipping company (waybill) on it and put clear packing tape over it.

I don't have that guys packaging, but if I had to guess I would go with ran over it with a forklift. The boxes those come in don't get crushed in the sorting areas, even by very heavy loads. Some can even be stepped on or worse with no damage to the box, let alone the stuff inside. To bend that into an L you are going to need something extremely heavy, something like a forklift, a car, or one of the other transportation devices.

Depends on the company. Usually, its the customer's job to sort out what to do about it. There isn't much the shipping company can do about it except finish the shipment. In some cases the package may be rendered undeliverable, at which point I forget what is done, but that isn't the case here.
Shipping companies would actually rather sweep something like that under the rug than let it get back to their client that it happened. It doesn't really take many of of them to loose a client. Do it often enough and you will start loosing clients that you haven't even done it to yet.

At any rate, going by personal experience, I can state that shit like this happens with both outbound and inbound packages, although at a relatively lesser rate altogether.
Thanfully because of my own hobbies I never had to ship or get delivered to me anything of a larger size than a normal DVD case, but even so I had so much of a fill of receiving cracked videogame cases to discourage me from ordering anything voluminous from abroad.
Just to name one, the last mail-related incident I had was with a copy of Fallout 3 Collector's Edition I ordered from Play.com. I don't know how they managed to do it, but upon opening the package I couldn't help but notice that the metal case got warped due to two bumps it got while en route. A metal case, of all things.
I haven't contacted Play.com for a refund yet, but I think that the damaged case gives off some unique "I survived an atomic holocaust" feel. Still afraid to unwrap it and check if everything inside is fine, though.
