Ghosts and phantoms are different species in Touhou. A Phantom is an ephemeral spirit that drifts freely and lives in the Netherworld. A Ghost is the soul of a dead mortal, who has yet to cross the Sanzu and find their judgment.
Phantoms are created from mortals, but it's possible to die without creating a phantom, as well as to create multiple phantoms, or even create a phantom without dying. A human can only make one ghost, will always make one ghost, and that ghost will always go on to the Sanzu, perhaps after haunting somewhere for a bit. (Yuyuko is an exception: she cannot move on due to the way she died.)

A ghost looks like a human (Yuyuko). A phantom looks like a blob ("Myon", which is Youmu's phantom body and not an independent existence).
