It was approved by ZUN, true, but there are several other mistakes in ZUN-approved materials that still don't count because they were not ZUN-drawn. Like in SSiB, there's the appearance of Lily Black, and in OSP, where Momiji appears with wolf ears and a tail. He probably only tells the artists what to draw and doesn't check up on them a whole lot.

ZUN's supervision has changed depending on which work it was; the direction he gave Aki Eda is basically nothing compared to the direction he gives Harukawa. Plus, ZUN puts importance on certain details (like who cameos), while other details he'll ignore entirely (like architecture).
