I'm guessing that the Scarlet Devil Mansion has indoor plumbing, so I can see them using modern dental hygienic instruments. Same goes for the Tengu and Kappa of Youkai Mountain.
The rest of Gensokyo may use older methods of dental care (such as older horse or boar hair toothbrushes), but no modern conveniences to assist them.

The human and the civilized ones that don't live in the wildness may use those boar/horse hair toothbrushes you mention, but it's not like you need plumbing at all to make use of those.

Yukari probably uses some modern electric ones.
