Honestly, I'm still trying to figure out how I beat the game in the first place. I think it took me about 6 months, and it was my first Touhou game.

Pattern difficulty-wise Remilia solely because of Scarlet Meister, but you can bomb her and Meister doesn't take all that long.
Survival difficulty-wise Yuyuko probably since you absolutely have to learn many of her cards and she takes a lot of lives, which you usually don't notice because PCB gives you a ton of lives and bombs. Imagine if Yuyuko was in say, UFO with the same patterns, she'd be a nightmare.
All capture-wise obviously Kaguya due to the sheer endurance requirement.

I'm still playing in easy mode and hasn't finished yet this game.. I'm sorry, please don't be mad at me.

I refuse to play easy, even for the spell cards. My roommate is a Touhou guru and his only challenge now is to time out Flandre's final spell. I would be ashamed if he ever caught me playing easy

Are you serious OP? Remelia is the most fun and easiest boss for me to beat. I could 1CC EoSD on hard if it wasn't for some of Sakuyas cards in stage 5.
The only thing I need to bomb on normal is sakuya and that demon mountain devil whatever it's called card Remilia has. The afterimage fucks with me.

No miss on Normal:
Yuuka, Shinki, Remilia, Kanako and Miko are easy
Yuyuko, Kaguya and Byakuren are medium.
Okuu and Yamaxanadu are hard. (Yama is easy when using Medicine)
3 fairies and Yumemi are very hard.
Haven't tried Mima and Sariel

On Lunatic, Remilia, Kaguya, or Byakuren, probably.

I only play normal and (rarely) hard, so my opinions are probably different to lunatic players.

I think that Eirin/Kaguya are probably the most difficult if you aren't bombing. Due to the deathbombing mechanic they're not too tricky if you're going for a 1cc at any cost, but I find their danmaku to be some of the hardest. Apollo 13 is pretty much impossible for me.
