Zun can't draw men, muscle tone, or proportions. (He can't really draw women either, but he can at least hide them in baggy clothing, big hats, and long skirts.)

But a male hero in a game could not be a blob in baggy clothing. He would have to be drawn as a traditional shounen protagonist for the game to be commercially viable (we are talking the early days here) and this is beyond Zun's artistic abilities.

There was a chance to introduce a male protagonist in the early days, assuming someone could draw such a character for Zun, but touhou rapidly evolved to the point where such a character would be unacceptable to the fanbase.

Rinnouske... plus, Marisa's father is still alive, but probably not on speaking terms with her, Youmu's grandfather is probably still alive somewhere, and there are plenty of men in the human village. The moon isn't barren, either; at the very least the Watatsuki's manor has male guards, and the sisters themselves are/were each married to a grand-nephew of Eirin's (which means she either had or has a sister that probably looks somewhat like her, hopefully one of the written works will account for this some day).

Plenty more men have gone without mention or even creation so it's just a matter of if god-ZUN will put them in his stories.
