Casual gamers don't play Touhou because they like the game itself, they just play it because they're into Touhou itself. The hardcore gamers play Touhou because it's generally a hard game that casuals can't touch on harder difficulties.

I play Touhou and enjoy it very much, simply because I'm a fan of hard games, since most console games these days are easy as fuck. I enjoyed Okami, I'm a huge fan of the Zelda franchise so it was expected. It however was extremely linear and easy to beat, so while I enjoyed it, it's missing what Touhou gives me, a challenge.
