That's not Shiki.

Shiki became one of Roa’s host before he moves on to SHIKI’s body. The incident in the hospital where it reeks of blood and seemed to have a vampire incident had Shiki murdering the people in it when the body possession awakened his murder impulse, that’s why he unconsciously thinks he’s a freak beyond saving. This is also one of the points where he and Arc can relate to each other because Arc murdered every last of her brethren the moment her bloodlust awakened, which also explains the weird dreams in the prologue. It’ll come altogether when it gets revealed in Red Garden. Nasu has been prepping up Roa to be Shiki’s antithesis and the parallels between Shiki and Arc’s “fated meeting” since KT. My only gripe would probably be that he is a bit too lenient and obedient as a character, his arc with Alice and Aoko also doesn't really evolve that much during the course of the game. Furthermore, one aspect of the importance of noting Shiki's response is that he had been characterized as especially squeamish and disliking the sight of blood. Furthermore, because the flashback doesn't affect Shiki and Shiki's perception doesn't affect our understanding of it. Consequently, the only purpose the dream serves is getting Ciel catatonic. That has it's own problems, although they probably mostly just comes down to opinion. The two remaining purposes of Arach's drug are setting up a fight scene and giving Noel a power trip.

The are other ways, but I can't think of anything better. On the contrary, I don't view it as a problem at all, I think it's rather ingenious. It serves so many different purposes at the same time. Turning Noel into a DA is the finale of the character conflict between Ciel and Noel, allows you to showcases Ciel's guilt + the horror, shows some new background lore/plot details (the ritual, dark six etc.), AND lets you have a cool fight. I kneel Nasu; I couldn't do a better job.

Turning Noel into a Dead Apostle is the point though. The story is set up to make you think that Noel has been turning into a DA as a parallel to Shiki being Roa'd and struggling with her morality and sanity, only to rugpull and reveal that she actually hasn't. Then she really turns into one and they get to take what she was doing all of Ciel route hunting the mostly innocent to it's logical conclusion by reversing the human-vampire roles.
It's contrived but it's kino so I don't mind
