For all the jokes about this we don't actually know what the outside world's USA is like. From some hints in the print works and some of Sumireko's comments the outside world actually seems pretty dystopian. I think it was mentioned in the hifuu stuff that moon tours are pretty common, and that most people seem pretty unhappy. For all we know they may have sent an actual invasion force or something.

I'm forgetting the exact words, but PoFV also mentions a massive amount of people died suddenly in the outside world and hadn't even realized they'd died/knew what killed them, which seemed to be an allusion to a nuclear war (granted this may have just been a Hiroshima/Nagisaki reference).

It'd be pretty cool if we ever got a game that had some sort of conflict between characters of the outside world and Gensokyo. Or at the very least I hope ZUN expands on it.
