My pet theory is that losing her shrine to Kanako drove Suwako batshit insane and despite being older than Kanako now truly believes she is both a little girl and a frog, and being a god, her appearance changes to match her self-perceptions.

My theory is that, as a goddess, she can control what age she appears. Or, merlin style, she might age backwards. Or she was a poor village girl who was supposed to be sacrificed to the curse gods to appease them, but instead she gained control of them and stopped aging at that point.
