I think it was less a story and more Nasu trying to work through his fetish regarding raped women. I read Tsukihime first and I thought his portrayal of Kohaku was very good, primarily focused upon the mental aspect of her abuse, and it made me sympathetic to the issue in a way that I had not been before. But his portrayal of Sakura is almost entirely exploitative, i.e. it's concerned with physical details about her abuse, which it piles on continually throughout the route. It uses magic to make the situation as extreme and ridiculous as possible, and then keeps going. Oh yeah she's her brother's sex slave too haha! I think he has an emotional complex regarding this, which is why it keeps popping up in his work, and he wanted to make the most abused, impure woman possible so that he could try and deal with that at its extremes. Sakura is not a character, like Kohaku, and her "story" has nothing of meaning to say on the issue, she's just a tool created for his rape fetish. It also made me realize that was probably what was going on in Tsukihime, but he was restraining himself and so it was a fluke that it came off being tasteful in the end.

Kohaku's rape doesn't feel fetishized because it's never brought up in that way. When Sakura's rape is explained, it's all about how dickworms made her all hot and bothered, and how she was perpetually horny, and how she's an educated prostitute now. When Kohaku's rape is explained, it's described in a completely non-sexual way - a violent psychopath assaulted an 8-year-old girl, both sexually and psychically, until she broke down. We don't get any description of how it was done, because it would be distasteful and unnecessary. It makes Kohaku's situation seem far more realistic.
It's also reflected in how both heroines act with the protagonist. Sakura is a goddamn educated prostitute (I will never get over this shit, seriously), while Kohaku actually has no clue what sex is. She was used like a tool, and never felt any pleasure from it. For her, making love is a concept more foreign than for any other heroine in Tsukihime, and it's believable.

They'd end the previous episode with Sakura unleashing Berserker on them, then spend the next episode on the chase. In the Fate route's bridge scene, when Shirou glances back at Saber after she told him that she doesn't care about him, for a split-second we're shown this sprite that is only used in this scene. Takeuchi made an entire separate sprite just for this, to make sure the player (and Shirou) understands that Artoria is lying, and how she hates herself for saying such a thing. Any other artist would've probably just reused the same "hanging her head in shame" sprite she has, but Takeuchi felt that it wouldn't suffice for such a crucial emotional moment.
This is the kind of dedication that makes FSN as good as it is.
