I have a sixaxis controller.

I want to play TH12.3 with it.

I got the controller working and all that, but I can't seem to get the D-Pad to be responsive for movement. Only the analog stick... Does anyone have a solution perhaps?

Set your own however you like it. Popular controls are square for A, X=B, then triangle or O for C, and R1 for dash. You change controls by going to profiles, 2nd option, pick the profile.

In the older Touhou games they mapped fine.
Newer ones you need a program like x-padder.
When I used Joy2key the piece of shit wouldn't let me do diagonals.

OP, you should be able to do Control Panel -> Game Controllers, select the controller from a list, go to properties, and switch the analog and D-pad axes.

SIMPLE ANSWER USE JOY2KEY to configure movement ONLY, and leave the buttons as you want them.
the reason you can't normally use the d-pad, is because the PC detects up,down,left, and right as BUTTONS not DIRECTIONS.
where touhou 10.5 and 12.3 both attempt to set all four directions automatically
