Stage 1 should be her in caterpillar form, then she makes an appearance as the stage 5 mid-boss in her butterfly form.

So the stage one boss appears to be some sort of butterfly or moth youkai.
Any ideas as to what it's alluding to? She's no doubt unrelated to the incident, but it's still fun to speculate on what she could be based off of, assuming she's not just a generic beast youkai like Mystia. Doubt it's the Mothman, as western "youkai" typically don't appear in Gensokyo out of nowhere. The DDC cast are all based off eastern youkai, so don't bring them up.

The demo cds all have stage 1 characters. The only two exceptions are Kyouko and Mystia. It could be stage 2 but it's probably 1

Now that I think about it, TD might be an exception. It would be very strange for fans to see Yuyuko on a demo disc, of all characters. Kyouko could have been used in that case to keep the surprise.

Kyouko I can understand, but I wonder why Mystia breaks the pattern? I mean, IN was the first mainline game in the series to have a game cover that featured a final boss instead of the extra/phantasm boss, as EoSD and PCB did. Maybe ZUN originally intended to switch up the demo discs too?

Well the whole idea is of minor things causing a major effect.

Wouldn't it be interesting if the first boss somehow was behind everything but it wasn't realized at first? Like her actions set off all the other stuff that happens in the game. Back in EoSD, ZUN said he gave Rumia a power like control over darkness because it was strong sounding but she was a weak first boss. If this boss had a minor power but it cause all the other problems in the end, it'd be a similar kind of joke. Although ZUN is kind of predictable so most likely she will be throwaway or be like Kogasa or Chen and show up in th extra.

Both Aya and Cirno have been in bunches of games, and had their own spinoff games as protagonists, and were previously in PoFV as protagonists, a full numbered so-called mainline game. Technically Aya was also a protag-assistant in SA. Nothing about them is surprising. P2 Reimu not being in is what's surprising. Maybe it's her turn to be a stage 4 PC as enemy boss.
