I'd imagine it largely depends on where you are. In Shibuya? Definitely real people. Akihabara? 15/35/50 real person/fashion freak/actual otaku. Other Tokyo areas lean heavily towards real person. Other Japan leans toward fashion freak, with some real people and otaku. Non-Japan anime convention is 97% crazy people, 2% otaku, 1% employee. Non-Japan non-anime convention is 100% crazy people.

They almost always look shitty in real life, usually because there's this stereotype around them being "sexy" so the girls who tend to wear them come off as slutty and/or tryhard "omg so kawaii X3" weeb types. They look decent on Asian schoolgirls sometimes, but it requires a delicate balance of skirt/sock ratio and overall look. More often than not it just comes off as some fetish-cosplay attentionwhore look with loud flashy rainbow stripes and a tiny lewd skirt.

They're classy and more understated than thigh-high/overknee socks (especially the colorful/striped ones). They're also less played out by basic weebcore sluts.
