Alice prefers fighting with honour and never resorts to playing dirty, but she fights all out at all times. The only secret weapon she has is the grimoire, which - and you know this - will probably never be opened again.

Whether that's true or not doesn't matter since as long as she's bound by danmaku law, Keine is fairly harmless. Outside it she's pretty much godlike, but it makes no difference in a fight.

Realistically (in Touhou? Yeah, right...), the danmaku law would have to be very old, probably dating back to the founding of Gensokyo. The more powerful youkai must've realised pretty quick they couldn't go around wielding godlike powers any way they liked when confirmed to a tiny little spot of wasteland. The Komeji are satori, a sort of youkai that is particularly out of place in the modern world, so they (or their clan) may have been among the first settlers of Gensokyo. They'd have had plenty of time to adapt to the rules.
As for how the Lunarians know of it, it may have been adopted after Yukari's "failed" moon invasion. After all, the lunar capital is an environment not all that different from Gensokyo - not a lot of room, fairly small ecosystem, and you really don't want to attract attention from the outside world.

As far as I can tell, the only one who uses danmaku without having any prior knowledge of it is Flandre, having been sealed away for hundreds of years, and with the SDM only arriving recently.

Tenshi is not as powerful as you have her. She's around Kaguya/Mokou level. Also, Evil Eye Sigma is Okuu level.

The weird thing about Koishi and Okuu is that they LOST their powers by the end of the game, Koishi gaining Satori level powers and Okuu becoming Stage III material.

Koishi didn't so much lose her power as "power down". She could go back to her in-game level if she decided to keep her third eye closed again, but she won't. She simply, with some vague sense of certainty, won't.
Think of it as how characters in RPGs always have some special power when you encounter them that they don't have once they become playable.
