The ones that don't walk you back maybe die for real, but they'll respawn within a couple of days anyway. It's still improbable they die after a battle though, sicne danmaku was created to avoid that in the first place.

And before you ask why Mokou died a load of times you can easily change the rules on what type of danmaku you will use so the 2hus just used deadly bullets there.

Or maybe actually part of danmaku rules is that they put some kind of barrier or protective aura on themselves that protect them from any harm. That's why they can withstand thrown keystones and rocks, and physical attacks like sword slashes and punches, but in exchange it'll drain their fatigue and magical energy that sustain the barrier and possibly the ability to produce normal bullets or danmaku, thus end the duel if it's completely depleted.

In Mokou's case, she didn't put on the barrier and let the lasers and bullets incinerated her body.

It's more probable that it's the danmaku that are dumbed down. Since danmaku was born to give weaklings without powers a chance of winning. Also it's known danmaku don't kill but just get your morale down, but I understand your worrier about stuff like Tenshi's keystones and heavy physical contact. She probably only uses them against worthy opponents or something.
