Why haven't the villagers revolted over Reimu refusing to actually exterminate anyone? It has been shown that they are capable of handling things for themselves by teaming up.

Each new Hakurei maiden is a orphan who is adopted by her predecessor. The Haurei maiden cant have family ties that might interfere with her being the mediator between humans and the supernatural.
Also until the Moriya arrived the Hakurei shrine was the only Shinto shrine so part of the maiden's super powers is that she could channel the abilities of the entire Shinto pantheon. Reimu's limitation is that she hates to study and has never learned the entire Shinto pantheon.
Its unclear if the Miko have any powers that are hers and not loaned to her by the gods.

The thing is, the fact that the gods doing it means that Reimu can't actually abuse her powers: She tries something funny, and she gets dropped like a paladin switching alignment, losing ALL of her powers. In a justified case, like defending her life from an angry mob, she can go all out.

Yes- and no- ZUN only sold roughly 100 copies of DiPP in the initial version. Then friends told him to tune down the horror and that it was too dark. It was re-released with a new liner story the very next Comiket and sold out.
The episodic new liner story does not, however, contradict the older story. It merely avoids it.
